  From here you can access top quality forms, free of charge. However you do need to read and agree to the following conditions.

If you are a private individual, simply wishing to use the forms for your own use, then you are welcome to print as many as you like, and we hope you find them useful.

If you are a professional, for example a clinical psychologist, psychiatrist or psychiatric nurse, then you can print as many as you like for your own professional use. You may not however modify them or pass them off as your own.

If you are an organisation, for example an NHS Trust, then you can print as many as you
like; thousands if you wish. Again though you may not modify them in any way, for example
by putting your own logo on them or passing them off as your own.

In no case may you sell them on.
